Piston twin, heavily loaded, with possibly a gusty tailwind. Get behind the power curve, and be a little late on the go, and I can see it happening.

Especially if you forget the Flaps gear flaps part...(not saying that's what happened).

Back at my old airline I'd routinely add an ask in the dispatch release if the flight crew could shoot a CAT II if/when able. Maybe an idea going forward would be for me to ask PF in an ACARS to just throw a go-around in? Sure if it's briefed out it's expected , and wouldn't be real-world, but is it something that's worth considering.

Letting me be the one to explain the extra bits of fuel consumed, that's no issue at all. I like my flights done safely.
