Adding /clearing MELs/CDLs

Not too sure the exact name but I’m usually given a printed page from the MEL/CDL book that has the DMI number on it and the operational/maintenance procedures associated with the Mel/CDL that was added. Also, our computer program will notify us of something has been opened or closed in regards to DMI’s added/removed

I know some airlines have a form and some airlines take a page from the MEL book

but I think theres a specific name for it

For example : ferry permit is M29
 
First shop I worked at had MXC write the info on a “deli slip” and they had to hand it to the dispatcher.

At my current shop all I get is a screen printer message for any MEL/CDL change across the entire fleet.
 
We get alerted through the flight planning system of an addition or removal and/or a phone call from MX CNTRL. They have specific forms for ferry flights and test flights.
