Access to prior 8500-8s, and reporting prior doctor visits.

Dr. Forred,

I'm looking to reapply for a new first class medical this year. I know on my 8500-8, I need to disclose the past 3 years of doctors visits. I had a carbon copy of my last 8500-8 which was paper, but I have since then misplaced it. I was going to use that for making sure I reported all doctor visits in the prior 3 years. I can really only remember my doctor visits over the past 2 years. Is it important to accurately re-report doctor visits that were already reported on my last 8500-8 that was done less than 3 years ago from now?

If I need those records, is my best bet going to be to call my AME and get a copy of my 8500-8 they have on file?
 
Thanks for the insight. I called my previous AME to try and get that information, but they would not release it... Very frustrating. I'm out of town, and I am planning to go to a different AME.
 
You can request them from the FAA. I got my old forms that way. If I recall it takes a few weeks.
 
When calling my prior AME today, I was told that for previously reported doctor visits we can just put previously reported, and then just add any new doctor visits that have happened between our last medical and this next one. Is this true?
 
Ok thanks. It's interesting an AME would say otherwise like that. I'll just request it from the FAA. Thanks for the help guys.
