upcoming medical and DUI/dropped charge question

Greetings!

Ok when I was first arrested for suspicion of DUI i sent in the required form and got a nice reply from the FAA saying thanks, dont do it again.

Its been quite a while since my last medical however, and I don't recall what exactly I have to put down now on my medical... is there a form I need to bring with me?

The final disposition of the case was the DUI charge was dropped. Does this affect anything?

Just report it in box 18v and tell the AME about it. He will write a comment in Box 60. If it was reported before, it is no big deal.

gotcha, yeah i know its one of those "previously reported, no change" type of deals... just couldnt remember if i needed an additional form this time around
