Accident occurred Thursday, August 14, 2008 in St. Louis, MO
While en route to the destination airport, the first officer (FO) warns that his flaps 45 degrees landings are “a little extreme.” The FO continues by stating that the landings are “a little non-standard, but it’s way more fun.” The flight data recorder (FDR) showed an increased in the airplane’s pitch angle at approximately 34 feet above ground level. The landing flare continues to increase to approximately 13 degrees of pitch angle with a corresponding angle of attack of 17 degrees. The airplane’s stick shaker/stall warning system activates seconds prior to the airplane’s landings. During the landing sequence, the airplane’s tail sections contacted the runway.

I was wondering the same thing when I was reading the report for the first time. I know the FO. Still here, surprisingly.
 
They have the CA's times listed but not the FO.... 13K with 4K in type. Still.... awefully strange...

THE FLIGHT CREW REPORTED THAT AS THE AIRPLANE WAS CLIMBING THROUGH FL 310, THE FLIGHT ATTENDANT REPORTED A LOUD CRASHING NOISE IN THE REAR OF THE CABIN. ADDITIONALLY, A PASSENGER REPORTED SEEING A FLASH FROM THE LEFT ENGINE. THE CAPTAIN STATED THAT HE VISUALLY CHECKED THE LEFT ENGINE AND SAW A HOLE IN THE COWL. THE CAPTAIN ALSO REPORTED THE LEFT ENGINE'S N1 AND N2 RPM AND INTERTURBINE TEMPERATURE WERE LOWER THAN THE RIGHT ENGINE AND THAT THE AIRPLANE WOULD NOT ACCELERATE TO CRUISE SPEED. THE FLIGHT CREW KEPT THE LEFT ENGINE RUNNING, BUT ELECTED TO RETURN TO ST. LOUIS, MISSOURI AND DECLARED AN EMERGENCY. THE CREW REPORTED THAT WHILE THEY WERE APPROACHING ST. LOUIS AND RETARDED THE POWER LEVERS TO IDLE, THEY HAD THEIR FIRST INDICATION OF VIBRATION FROM THE LEFT ENGINE. THE CREW STATED THAT THEY THEN SHUTDOWN THE LEFT ENGINE AND LANDED WITHOUT FURTHER INCIDENT. NO INJURIES TO CREW, 49 PASSENGERS & 2 LAP CHILDREN.

Narrative: (-23) AIRCRAFT N814HK, WHILE ON APPROACH TO LAND AT LAMBERT-ST. LOUIS INTERNATIONAL AIRPORT (STL), SUFFERED SUBSTANTIAL DAMAGE TO THE REAR FUSELAGE SECTION OF THE AIRFRAME WHEN IT CAME IN CONTACT WITH THE RUNWAY SURFACE. INVESTIGATION REVEALED THE AIRCRAFT FINAL APPROACH ANGLE OF ATTACK AT TOUCHDOWN WAS EXCESSIVE CAUSING THE AIRCRAFT TAIL TO STRIKE THE RUNWAY CAUSING SUBSTANTIAL DAMAGE.

Not the first time this thing has had issues... but the first time it's due to crew.
